Heated Tobacco vs Nicotine Pouches: Inhaled vs Oral, Honestly

By the iqoser team · Updated · 5 min read

A pack of TEREA sticks for heated tobacco

Heated tobacco and nicotine pouches are both pitched at smokers who want something without smoke, and that is roughly where the similarity ends. One is inhaled, the other sits in your mouth. One is made from tobacco leaf, the other is usually tobacco-free. Both contain nicotine, and neither is a quit-smoking treatment.

We sell heated tobacco and not pouches, so we have tried to keep this even-handed. The first point applies to both: quitting nicotine completely is the best thing you can do for your health. India's National Tobacco Quit Line (1800-112-356) is free, and our quit smoking guide sets out the options.

How each one works

Heated tobacco. A device such as the IQOS ILUMA i ONE heats a stick of real tobacco without burning it. You inhale the resulting aerosol into your lungs. A session lasts a set time, on ILUMA up to 6 minutes or 14 puffs.

Nicotine pouches. A small, pre-portioned pouch, usually made of plant fibres, nicotine, flavourings and fillers, is placed between the gum and upper lip. Nicotine is absorbed through the lining of the mouth. There is no device, no heat and nothing to inhale. Most pouches sold today are described as tobacco-free; the nicotine is the tobacco-derived part.

Five differences that matter

  1. Route. Heated tobacco goes into your lungs as an aerosol. Pouches deliver nicotine through your mouth. That is the biggest difference, because inhaling anything brings your lungs into the picture.
  2. What's in it. TEREA sticks are processed tobacco. Most pouches contain no tobacco leaf.
  3. Equipment. Heated tobacco needs a charged device and compatible sticks. Pouches need nothing but the tin.
  4. People around you. Heated tobacco releases an aerosol into the room, which is not the same as smoke but is not nothing for bystanders. A pouch releases nothing into the air.
  5. The habit. Heated tobacco keeps the hand-to-mouth, inhale-and-exhale ritual of smoking. Pouches replace it with something closer to chewing gum, used discreetly over a longer period.

Who tends to use which

People who use heated tobacco are usually smokers who want to keep the physical ritual and the taste of tobacco while losing the smoke, ash and lighter. People who choose pouches are often those who want no inhalation and no smell at all, or who need something discreet. Neither group is better or worse; they are looking for different things. Which product people pick is a preference, not a health ranking.

What's known, and what isn't

Heated tobacco. Studies, including Philip Morris's own and some independent ones, measure much lower levels of many harmful chemicals in the aerosol than in cigarette smoke. Lower exposure has not been shown to mean lower risk of disease, and it is not risk-free. The World Health Organization's statement on heated tobacco says reducing exposure does not render these products harmless, and WHO does not endorse them as a way to quit. Our heated tobacco explainer covers this in detail.

Nicotine pouches. With no burning and no inhalation, the chemicals in smoke aren't part of the picture, but the nicotine is. In its first report on nicotine pouches, published in May 2026, WHO described nicotine as highly addictive and said pouches are not risk-free products. Effects on the mouth and gums and the long-term effects of daily use are less well studied than smoking.

For both: neither has decades of data behind it. Anyone who tells you exactly how risky either one will turn out to be over thirty years is guessing.

What neither one is

  • Not a quit aid. Neither heated tobacco nor consumer nicotine pouches is a medicine for stopping smoking.
  • Not nicotine-free. Both contain nicotine, which is addictive.
  • Not risk-free. Not for you, and in the case of heated tobacco aerosol, not for the people around you.

How pouches differ from nicotine gum

This trips people up. Nicotine gum, lozenges and patches are nicotine replacement therapy: medicines designed to help you stop smoking, used for a limited period on a reducing plan, and sold in Indian pharmacies. Nicotine pouches look similar in idea but are consumer products, not a quit treatment. If what you want is to stop, our nicotine replacement therapy guide is the place to start.

If you are weighing them up

Ask yourself what you're really trying to do. If the answer is "stop smoking", the route with the best evidence is a quit attempt with NRT or prescription medicine and support. If the answer is "I'm going to keep using nicotine and I want no smoke", know that both options carry real unknowns, that switching completely matters, and that using either alongside cigarettes keeps you exposed to smoke. Our overview of smoke-free alternatives in India puts all the options in one place.
