Heated Tobacco vs Vaping: What's Actually Different

People often lump heated tobacco and vaping together, and they do have things in common: no flame, no ash, an electronic device, and nicotine. But they work on different materials. Heated tobacco heats real tobacco leaf. Vaping heats a liquid. That one difference shapes almost everything else.
Before the detail, the most useful fact: the best choice for your health is to quit nicotine completely, and neither of these is a quit-smoking treatment. If you want to stop, free support is available on India's National Tobacco Quit Line, 1800-112-356, and our quit smoking guide for India explains the options.
How each one works
Heated tobacco
A heated tobacco device, such as the IQOS ILUMA i ONE, heats a stick of processed tobacco to a temperature that releases nicotine and flavour without setting the tobacco alight. In ILUMA's case, each TEREA stick contains a sealed metal element that the device heats by induction. What you inhale is an aerosol made from heated tobacco. Our heated tobacco explainer covers this in more depth.
Vaping
An e-cigarette, or vape, heats a liquid, usually containing nicotine, flavourings and carrier liquids, on a heating coil until it turns into an aerosol. There is no tobacco leaf in the device. The nicotine in the liquid is the part that came from tobacco.
Side by side
| Heated tobacco | Vaping | |
|---|---|---|
| What gets heated | Tobacco | A liquid |
| Contains tobacco leaf | Yes | No |
| Nicotine | Yes, from the tobacco | Usually, added to the liquid |
| Burning | No | No |
| What you inhale | An aerosol | An aerosol |
| Consumable | Tobacco sticks | Liquid, pods or cartridges |
| Flavour comes from | The tobacco blend, plus menthol or capsules in some sticks | Flavourings in the liquid |
| Risk-free? | No | No |
What they have in common
- Nicotine. Both normally deliver nicotine, which is addictive.
- An aerosol, not "just vapour". Both produce an aerosol of fine particles and droplets, not harmless steam. "Vapour" is a misleading word.
- People around you. Both release aerosol into the air around the user. It is not the same as smoke, but it isn't nothing.
- Limited long-term evidence. Both are relatively new compared with cigarettes, so there is far less long-term data on what regular use does over decades.
What the evidence says, in general terms
On heated tobacco: studies, including Philip Morris's own and some independent ones, measure much lower levels of many harmful chemicals in heated tobacco aerosol than in cigarette smoke. Lower exposure has not been shown to mean lower risk of disease. The World Health Organization's statement on heated tobacco says reducing exposure does not make these products harmless or translate to reduced risk, and that some toxins have been found at higher levels in heated tobacco aerosol than in cigarette smoke.
On vaping: WHO's questions and answers on e-cigarettes state that e-cigarettes are harmful to health and are not safe, and that, as consumer products, they have not been proven effective for quitting at the population level.
There is no honest way to rank one as "safe". Neither is.
The difference that matters to users
If you strip away health questions, the experience differs in predictable ways:
- Taste. Heated tobacco tastes of tobacco because it is tobacco. Vaping tastes of whatever flavour is in the liquid.
- Session vs continuous. An ILUMA session runs for up to 6 minutes or 14 puffs per stick, then it ends. Many vapes can be used continuously, which makes it easy to lose track.
- What you buy. Heated tobacco uses sealed tobacco sticks. Vapes use liquids or pods.
Where quitting fits
Quitting completely is the best thing a smoker can do. The methods with the strongest evidence are:
- Nicotine replacement therapy: gum, lozenges and patches, sold in Indian pharmacies. Our NRT guide explains how to use them.
- Prescription medicines such as varenicline or bupropion, through a doctor.
- Counselling and support, including the National Tobacco Quit Line (1800-112-356) and the mCessation programme (give a missed call to 011-22901701).
Heated tobacco is not a quit aid, and WHO does not endorse heated tobacco or e-cigarettes as consumer quit methods. If you smoke and won't quit, switching completely from cigarettes to heated tobacco reduces exposure to many harmful chemicals, but it is not risk-free. For an overview of every smoke-free option you are likely to hear about, see our guide to smoke-free alternatives in India.
Questions
Is heated tobacco the same as vaping?
No. Heated tobacco heats real tobacco; vaping heats a liquid that usually contains nicotine and flavourings.
Do both contain nicotine?
Heated tobacco does, and most vape liquids do. Nicotine is addictive.
Is vapour from either one just water?
No. Both produce an aerosol of fine particles and droplets, not steam.
Is heated tobacco or vaping risk-free?
Neither is. WHO considers both harmful, and the best option is to quit nicotine completely.
